INITIAL RESPONSE

When a major spill, presenting hazard to employees or the environment such as a broken vial or dropped syringe occurs:

• R - Remove individuals from danger and minimize exposure
• A - Alert/ call trained lab personnel, Radiation Safety Officer and then security at 77777
• A - Alert/ call trained lab personnel, Radiation Safety Officer and then security at (312) 227-7777
• C - Contain, stop the spill and isolate the area
• E - Evacuate the area

SECONDARY RESPONSE

& FOLLOW UP

• Survey area for contamination as directed by the Radiation Safety Officer
• Dispose of decontaminated material as directed by Radiation Safety Officer
• Complete Safety Event Report
