INITIAL RESPONSE

When you witness an infant or child being removed from the area by a stranger or when an infant or child is missing from the Medical Center:

  • Initiate “Missing Patient Alert” (formerly Code Pink) by calling 74444 to connect with an Access Coordinator and Security Services simultaneously
  • Initiate “Missing Patient Alert” (formerly Code Pink) by calling (312) 227-4444 to connect with an Access Coordinator and Security Services simultaneously
  • Provide your name and location
  • Provide descriptive information about missing patient: age, sex, description of clothing/blankets, and last known location
  • Make overhead announcement for “Missing Patient Alert” via Nurse Call Station or Avaya phones to alert staff and families
  • Secure all entry/exit points in your area (including elevators/stairwells)
  • Search for missing patient

 

SECONDARY RESPONSE

& FOLLOW UP

  • Listen for “Missing Patient All-Clear” overhead announcement before resuming normal operations
  • Security Services will notify the Chicago Police Department, if appropriate
  • Security Services will prepare a report of the incident
  • Social Services will complete the Safety Event Reporting System (SERS)
